New Student Registration

 

TO EXPEDITE REGISTRATION, PLEASE COMPLETE THE FORMS LISTED BELOW

Registration is by appointment only.

NOTE:  Students returning to Manheim Central within the same school year, please contact the Business Office for information.

 
DOCUMENTS REQUIRED AT TIME OF ENROLLMENT
  • Proof of Residency*
  • Child’s Birth Certificate (Original)
  • Child’s Immunization Record (Link to Health Services listed below)

*One of the following documents is required to verify the parent’s/guardian’s name and address within the district:  Mortgage Document, Renter’s/Lease Agreement, or Utility Bill.

DOCUMENTS REQUIRED IF APPLICABLE TO YOU

  • Affidavit for Proof of Guardianship
  • Notarized Parent Custodial Agreement
  • Agency Letter if Foster (1305) Student
  • IEP (Document for students needing special services)
  • 504 Plan (Document for students needing special services)

**These forms are required only for students who are registering during the current school year.  If your student is registering for the following year, the forms will be completed at the beginning of the school year.
