| MEDICAL EXAMS
The Pennsylvania School Health Law requires children to have a complete physical exam upon original entry to school, in the sixth and eleventh grades, and when a student is transferring to Manheim Central. Attached is the Private Physician's Report Form to be filled out when your child receives his/her medical exam.*
DENTAL EXAMS
The Pennsylvania School Health Law requires children to have a dental exam upon original entry to school, in the third and seventh grades, and when a student is transferring to Manheim Central. Attached is a Family Dentist Report Form to be filled out when your child receives his/her dental exam.*
*The district will accept medical/dental reports completed within one year prior to a student's entry into the grade where an exam is required.
ATHLETIC EXAMS
A physical exam is required before a student participates in district athletics. The exam can either be with your family physician or with Physical Therapy Associates of Manheim during the district’s scheduled physicals. The Sports Physicals Schedule can be found under the Athletics link and the Athletic Exam Forms are available in the main offices of the High School and the Middle School.
